OWNER AGREEMENT, PET PERSONALITY & EMERGENCY CONTACT INFORMATION

Please fill out a registration form below for each pet who wishes to attend Lucky Dawg Dos Daycare:

FEES: Fees are based on a pass plan. or per day. A pass is valid 4 months from the purchase date.

HOURS & BOARDING: Sunday – Saturday from 7:30am to 6:00pm. There will be a $1.00 per minute charge for any dawg left after 6:30pm. Boarding inquiries are reserved through, Luckydawgdos@gmail.com. Boarding is 24hrs/ 7 days a week.

RESERVATIONS: Required. Cancellations within less than 24-hour notice will be charged full fees.

By completing this form, I hereby certify that:
My dog(s) are(is) in good health and have not been ill with any communicable conditions within the last 30 days. My dog(s) have not harmed or exhibited aggressive/threatening behavior toward people or another dog.

I have read and understand the following:

1. I understand that I am solely responsible for any harm caused by my dog(s) while my dog(s) is/are attending Lucky Dawg Dos Daycare. I agree to hold harmless & indemnify Anthony Roybal of Lucky Dawg Dos Daycare, any and all persons who might assist in the care of my pet, or from any claim for damages and/or injuries to my pet and property.

2. I further understand and agree that by admitting my dog(s) to the facility, Lucky Dawg staff have relied on my representation that my dog(s) is/are in good health and have not harmed or shown aggressive or threatening behavior toward any person or any other dog.

3. I further understand and agree that Lucky Dawg Dos Daycare and their staff and volunteers, will not be liable for any problems which develop, provided reasonable care and precautions are followed, and I hereby release them of any liability of any kind whatsoever arising from my dog(s) attendance and participation at the facility.

4. I further understand and agree that any problems which develop with my dog(s) will be treated as deemed best by staff and volunteers of Lucky Dawg Dos Daycare, at their sole discretion, and that I assume full financial responsibility for any and all expenses involved.
